Mobilizing for Action through Planning and Partnerships (MAPP)

MAPP is a way for communities to plan together to make sure everyone has a fair chance at being healthy. It helps communities figure out what health problems are most important and how to work together to fix them. MAPP involves getting lots of different people and groups in the community involved, like businesses, schools, and health organizations. It focuses on making changes to policies, systems, and the environment to improve health for everyone.

After going through MAPP, communities end up with a report about what health issues need attention (called a Community Health Assessment or CHA) and a plan for how to make things better (called a Community Health Improvement Plan or CHIP).
