The Madera County Department of Public Health (MCDPH) is holding COVID-19 Vaccination Clinics, using either Pfizer or Moderna vaccines; depending on supply.  Appointments and proof of eligibility are required.  If proof of eligibility cannot be provided, the appointment will be cancelled and can be scheduled at a time when proof can be provided.

Bring:	Proof of:  Living or working in Madera County Proof of age and/or industry of employment Proof can consist of:  Photo ID,  work badge, letter from employer business card, paystub.  Public  Health verifies proof visually, and does not keep information or copies.
Wear:	Short sleeve, loose fitting clothing that allows for easy vaccine administration
