Starting November 1, 2024, our office will begin renewing pesticide use permits (operator identification number and restricted materials permit) expiring on December 31, 2024. See below to download the form(s) necessary for your renewal.

To renew or apply for a pesticide use permit please submit the following:

1) A completed Permit Application Form (download form below).

2) A list or map of any crop changes, additions, corrections, or updates.

3) A completed/updated Mitigation/Alternatives to California Restricted Materials Worksheet if you have a California Restricted Materials Permit.

4) A Letter of Authorization1 must be submitted for representatives to make changes and/or sign the permit.

*All current Letters of Authorization are still valid until revoked by the property operator.

1Letter of Authorization is not required for Owner/Operators of permitted property.

Your completed pesticide permit application forms can be submitted in-person, by Fax: (559) 674-4071 or by E-mailMaderapue@maderacounty.com.

When the permit application forms are submitted, it will be processed in the order it was received. Once the permit is processed, an inspector will contact the applicant and discuss how to review and sign the permit.


Private Applicator Certification (PAC) is required for Restricted Material Permit applicants, anyone supervising California or Federally restricted pesticide applications, some pesticide applicators per label, and those who provide pesticide training to employees.

PAC holders with last names A-H expires on 12/31/2024. These individuals must take and pass the revised examination to continue certification. Please visit our website for links to exam study guides: https://www.maderacounty.com/government/agricultural-commissioner-weights-and-measures. CE classes are not valid for this renewal cycle.

As of January 1, 2024, all PAC holders (regardless of expiration date) must also possess a separate Burrowing Vertebrate Pest Fumigation (BVF) Certificate (in addition to the revised PAC certification) if they plan to use restricted fumigant pesticides for the control of burrowing vertebrate pests (i.e., aluminum phosphide). We will note on your permit when you have obtained the BVF Certificate.

*Attention: The PAC is no longer available or valid for household/residential use as of January 1, 2024. The PAC is only available and valid for individuals producing an agricultural commodity. In addition, the PAC cannot be used to fumigate stockpiles, the applicator would need to obtain a QAL or QAC category M for this work.
